What The UPTA Doing For The Industry

The United Physical Therapy Association exists to move the profession forward through direct advocacy, legal action, and policy innovation. Our current initiatives focus on addressing structural barriers that limit the impact of physical therapy within the healthcare system.

Why This Matters: Physical therapy is essential to maintaining access to care within the Medicare system. Yet outdated policies continue to undervalue the profession and limit its impact.

Petitioning CMS to Reform MPPR
UPTA has formally petitioned the Centers for Medicare & Medicaid Services (CMS) regarding the application of the Multiple Procedure Payment Reduction (MPPR) policy to physical therapy services.

MPPR reduces payment when multiple therapy services are provided during the same visit, despite the fact that these services represent distinct skilled interventions requiring separate clinical expertise and time.

Our petition asks CMS to reform the policy so that reductions apply only to duplicate services, not to different therapeutic procedures delivered during the same treatment session.

This change would better reflect the clinical reality of physical therapy and the value of comprehensive care delivered during a single visit.

Litigation to Allow Physical Therapists to Opt Out of Medicare
UPTA is supporting litigation challenging the federal policy that prevents physical therapists from opting out of Medicare.

Unlike physicians and several other healthcare professionals, physical therapists are legally required to participate in Medicare and submit claims for covered services.

This restriction limits practice flexibility and reinforces the classification of physical therapists as suppliers within the Medicare system rather than autonomous healthcare professionals.

The litigation seeks to restore parity by allowing physical therapists the same ability to opt out that other providers already possess.

Proposal to CMMI: Physical Therapy–Led Cost of Care Study
UPTA is developing a proposal to the Center for Medicare and Medicaid Innovation (CMMI) to lead a study examining the cost impact of physical therapy for specific medical conditions.

The study would evaluate the role of physical therapy in reducing total cost of care for targeted ICD-10 diagnoses, including conditions where conservative management has been shown to reduce downstream healthcare utilization.

The goal is to generate high-quality evidence demonstrating how early and appropriate physical therapy can reduce surgeries, imaging, opioid use, and other costly interventions.

This work is a critical step toward building future payment models that recognize the value physical therapy brings to the healthcare system.
