By submitting this form and payment of membership dues, I hereby agree to the following terms and conditions of membership in the United Physical Therapy Association (UPTA):
- Membership Dues
- I understand that the membership dues are required for active membership in the United Physical Therapy Association (UPTA).
- I acknowledge that membership dues are non-refundable, regardless of membership status changes, including termination, resignation, or cancellation.
- I understand that all dues and fees collected will be used solely to support the nonprofit mission of UPTA, including advocacy efforts, professional development programs, member services, and initiatives to advance the physical therapy profession.
- I understand that up to 80% of my dues may be used towards lobbying efforts to advance the mission of the UTPA.
- Commitment to UPTA’s Mission
- I affirm my commitment to UPTA’s mission to unite and empower physical therapists and related professionals in advancing the field through advocacy, education, and collaboration.
- I will support the collective efforts of UPTA to promote equitable healthcare policies, improve patient access, and address challenges such as reimbursement and prior authorization reform.
- Professional Conduct
- I agree to adhere to the ethical standards of the physical therapy profession and will conduct myself with integrity, professionalism, and respect in all interactions with colleagues, patients, and the community.
- I will comply with UPTA’s Code of Conduct, bylaws, and policies during my membership.
- Non-Transferability of Membership
- I understand that membership in UPTA is individual and non-transferable. I will not transfer or assign any of the rights, benefits, or obligations of my membership to another individual or entity.
- Membership Renewal
- Membership is valid for one year from the date of joining or renewal, and I understand that it will renew automatically, using the previous payment method, unless I notify the association in writing prior to my renewal date.
- Use of Member Information
- I agree to allow UPTA to use my contact information to provide membership communications, including updates on advocacy initiatives, educational opportunities, events, and other member-related services.
- I understand that my contact information will not be sold or shared with third parties without my explicit consent.
- Membership Termination
- UPTA reserves the right to terminate membership for failure to comply with its ethical standards, bylaws, or policies.
- I acknowledge that UPTA may terminate my membership for cause, including but not limited to violations of professional conduct or failure to pay membership dues.
